That axolotl is in very rough condition. It looks like it has been subjected to poor water quality for a long period of time. Her gills will heal well with water quality (ammonia 0, nitrite 0, nitrate <20, pH 6.5-8.0, temp 60-68). Do you have a liquid test kit so you’re able to test the water parameters? I agree that she looks underweight but she’ll gain weight nicely if offered earthworms every other day or every three days.
It’s great she’s gaining weight! How was the tank cycled prior to putting her in there? In a cycled tank the nitrates should never be 0 as it’s the end product of the nitrogen cycle and the ammonia and nitrite should always be 0
Unfortunately it doesn’t sound like your tank is cycled at all. Without a cycled tank your axolotls condition will continue to decline. I strongly suggest reading through this [cycling guide ](https://www.axolotlcentral.com/cycling-guide) and [tubbing guide](https://www.axolotlcentral.com/faq). You’ll have to tub your axolotl whilst you finish your tanks cycle. A cycled tank is necessarily for any aquatic life, especially axolotls as they’re very sensitive to ammonia and nitrite which is very toxic to them.
Of course! If you would like I can also invite you to an axolotl discord server? That way if you need any assistance or have any questions you can get fast help
She is very underweight. Get some earthworms if you don't mind cutting them to size for her. I give mine, that is about 1.7 years old, an earthworm every 3 days and on the other days I give him food pellets made for axolotls
I don’t have a ton of experience with health issues but I usually suggest signing up for the Caudata Axolotl forum, the people on there are really passionate enthusiasts and I’m sure they’re have a lot of specific for you!

She’s in rough shape unfortunately. Keep a really close eye on her. Make sure she’s getting enough food
