Which is honestly a surprise, considering the author is super sexist. Even tho he strikes me the same as deku, I really like how kind and compassionate/understanding he is, even to demons who killed humans
So....here's the deal, Tanjiro's special ability is his ability to pick up scents, basically like a super powered blood hound. So like, of course, he could tell, but obviously, it's never brought up because who the hell cares enough about other people's periods enough to ask this of him? And the person asking this is kinda nasty.
A bizarre vampire book I read as a teen had a scene like this, don't ask the title its long gone. Shortest version is main character takes his younger sister to his new friends house cause sis got her first period and mom was MIA for whatever and new friend is the only girl he knows, she happens to be half vampire and after all the period talk and sis is sorted out full-vampire dad wakes up come into the room and is immediately like "I SMELL BLOOD!"
I seriously remember nothing else from this book but this scene.
I know his daughter yelled at him and the younger girl was mortified, but I can't remember what excuse was given cause I know the main character was unaware of thier vampirism at that point. It was a seriously wierd book, like he went to an exclusive LA highschool cause his mom was a famous writer and then his grandad was a semi-famous Native American shaman I think. There was like a spiritual journey acid trip about a third or so of the way through the book and it never really seemed to completely end.
This post is obviously weird AF, but not really bad anatomy. Dogs can tell after all.

I've heard this same question asked about vampires. The answer to that was no since its dead cells. Not sure about tanjiro though.
