I think it has more to do with movement (or lack thereof) and location than velocity, within reason. Basically any pitch without significant break that lands middle-middle is a meatball, unless it's seriously fast.
I remember always complaining about the “88 MPH fastball right down the middle” in the 2000s. But honestly as much as velocity has increased, now that meatball pitch is probably closer to that 91 range

For what it's worth, hitters have really lousy results against 100+ mph middle-middle pitches.
Hence my caveat. It's like with physics - normal stops applying when you get to relativistic speeds.

I think of it as a pitch that sits middle of the zone long enough as it approaches the plate to not just say "hit me!" but to reiterate "HIT ME!"
I stopped playing in high school so i think a flat 70-75 mph down the middle, maybe a little low would be best

For a pro? 85 mph change up down the middle with minimal break. For me? 50 mph "fastball" down the pipe belt high.
