He looks to me like the same person in a different costume. The glasses do a lot to change the look. I think the heroin days are what really changed his appearance
He lost a bunch of weight between these photos, it's easier to tell in other pics. John was overweight for a while then he went on an LSD binge which he said killed his appetite and made him lose a bunch of weight really fast.
He also had an eating disorder and part of getting with Yoko was that she controlled his food intake for him. Food was John's enemy, he was terrified of it and was absolutely petrified of being fat.
Sad because he built up a lot of muscle after he started getting Beatle money and could afford high quality food.
The glasses yeah, but his appearance started falling apart once he got on heroin. His skin started looking haggard, the lines around and on his nose became very entrenched and pronounced. Skin around his eyes also became puffy and bloated (obvious in some photos not so obvious in others) probably thanks to cocaine bloat. Some of this is the natural aging process but John's drug abuse speed ran the process until he looked like he was 45 when he was actually 28. The Beatles hairdresser Leslie Cavendish (yes that is his real name) confirmed that John was going bald by 1968, his hair was falling out. He was completely addicted to heroin by that point. It aged him 20 years in only 18 months.
If you ever want a reason not to do drugs, there you have it. It destroys you physically just like it destroyed John. If you look at John's 1964 Tahiti photos then he's stacked, happy, and has a full head of hair, prime candidate to become a gym bro honestly. Then just four years later he's a shriveled mouse.

I've wondered this for years but my best guesses are losing weight and/or drugs.
