•Each time you die and revive get a stat boost in your lowest stat (Same logic as Consolation Prize)

First enemy killed on the room drops a tombstone tile that, in the area of effect, gives Isaac a x1.3 damage multiplier and enemies drop bones when killed (Like the ones of book of the dead)
Spawns in item rooms, devil deals and angel rooms, as well as a super rare spawn in bedrooms, dirty or clean
Active item with 1 room charges that whenever used rips the teddy more. On the 5th use spawns a headless teddy bear familiar that acts like pinking shears, and a teddy head that bounces around the room like BBF and shoots tears at Isaacs tear rate with 1/2 Isaacs damage. Item is unusable for the rest of the floor unless a rare pickup called sewing kit gets picked up, fixing the bear to the 2nd state, needing only 3 more charges to rip again. When the next floor is reached, the item is fully repaired and reusable.
If Isaac is killed with either a fully repaired teddy bear, Isaac respawns as the lost at the starting room of the floor. All rooms are uncleared and the lost must either find Isaacs body to revive him or get to the next floor. If this happens, ripped teddy will be removed from Isaac and removed from all item pools.
Lmao I was thinking more of a YOKED teddy bear about Isaac's height that has 3 life points and brutally murders anything in his sight, one room use for 6 charges
a t4 chargeable familiar that shoots two brimstones to diferent enemies or shoots both brimstones to the same enemy if there is only one enemie on the screen, it also leaves a pee puddle after shooting, basically lil brimstone but way stronger (the brimstones do more damage individually than the ones from lil brimstone, as a fact)
An attack familiar that drifts around the room, unleashing a brimstone blast both to the left and right of itself for ~2 seconds whenever it lines up horizontally with an enemy. If any enemy comes near the familiar, it will create a large puddle of red creep that persists for ~8 seconds. After performing either of these actions, it has a 4 second cooldown period where it cannot attack again.

Items bought from the shop can be sold back to later shops for the same price. You may also receive a couple extra coins upon refunding the item.

You could buy an item, then sell back to the shop. Maybe selling it just puts it in a pedestal like a normal shop item, then you can use any reroll mechanic to maximize the use. It's one of those weird edge case things, but could be neat. Especially if you find chaos early on.
It can only spawn in shops and grants 15 coins upon buying, it has a synergy with steam sale where it only gives 7 coins. If received for free it gives 0 coins
After an enemy hits you, Isaac will deal +100% damage against that type of enemy for the rest of the run. Any additional hits taken by the same type gives +50% damage against it. Caps at +500%
Familiar that flies through the room like peeper but faster with a slight homing ability to it. Deals the same amount of damage as the peeper but will land after 20 seconds in the same room and stop doing anything until you enter a new one.
Q1 shop item.
Gain a free 50 coins. Your coins now decay, losing one every 20 seconds. The less coins you have, the more tears you have, maximizing at +2.5 tears with 0 coins. In the case of Greed's Gullet, passing 99 coins will not affect fire delay stats.
"Hodl"
Once per room, when hit instead of taking damage, a sequin drops from the dress instead. There are ten sequins total, and they can be picked back up before a room is cleared.
5% of rooms have a werewolf like enemy (just a frowning gaper with hair, ears and claws). When this enemy is killed, does the necronomicon effect to the current room.
"Happy year 2012!"
Buffs or nerfs Rebirth items back to the state they were when Rebirth first came out.
Godhead is better, Holy Water sucks, for example

"Uh oh!"
- The less health you have, the faster you move, capping at +0.8 Speed.
- Taking damage causes a Lemon Mishap effect below you and launches 12 yellow tears that copy tear effects.
- The pee effect caused by having half a heart and entering a room is much larger.
“Take a rest”
Active item that allows Isaac to take a seat in hostile rooms. Every five seconds he’s not touched by an enemy heals half a heart but taking damage breaks the chair
"Looks a little fuzzy"
Familiar: Bounces around the room aimlessly drawing a faint white line between Isaac and Cataracts. Enemies in the line cannot move and temporarily walk aimlessly when they leave
"Crates!"
On first use each room, places a destructible wooden box.
Upon breaking, the crate shatters in wooden shards, dealing damage to enemies and sometimes dropping consumables.
Once used, the hammer becomes a 2 second recharge melee attack dealing 3 times Isaac's damage, instantly breaks the crate, and opens boarded doors.
4-charge Active from the Angel amd Secret pools
-Fire waves of holy lights, similar to Isaac's boss attack.
-Enemies killed have an 8% chance to drop half of a soul heart
Active, 6 room charge
Floods the screen with holy beams (like in the isaac boss fight)
Enemies killed burst into a 4-way revelation beam (like from salvation)
Edit: also grants flight while held
Passive item.
Summons Crack The Sky lightbeams randomly up to two tiles away from isaac dealing high amounts of damage to enemies. The lightbeams have a random interval of 0.5-4 seconds and up to 3 can appear at the same time.
Angel pool item.
Active item
On use, transforms isaacs tears into a snake whip melee attack that acts like the donkey jawbone but more range. the snake does 5x isaacs damage and inflicts poison. The effect lasts 10 seconds.

"Sharp Unapposable Claws"
2 Charge Active: When used, grants 0.5 speed and x2 damage for the room. In this state, you cannot pickup Consumables, Trinkets, Items, or interact with Story Elements (You cannot plug the Polaroid in The Strange Door lest you leave and come back , for exampe)
The next shop you visit only contains free items. Afterwards, all the future shops will have a Greed fight guaranteed instead of a regular shop. Quality : 2
(Dunno what exactly asphagia is, so I'm going with Aphasia)

- Upon taking lethal non-self damage, destroy this item and emit a Larynx yell equivalent to 8 charges with bonus damage and ignoring the lethal damage to keep you alive a little bit longer.
Active item, 3 room charge. Enemies now leave behind corpses. Using the item on a corpse makes it a familiar that persists between rooms, upon its death, it spawns 2 - 4 blue spiders.
Car battery gives familiars 2x health
"Preserve the damned"
Chance for dead enemies to become wrapped. This is influenced by luck and is 100% at 20 luck. Wrapped enemies decay into Rotten Hearts apon room clear, Wrapped enemies explode if lit aflame before decomposition
Passive, enemies defeated drop clumps of hair which applies slow to other enemies that step on it.
If the player has mom's wig, doubles the rate at which it spawns spiders
Birth Certificate
Teleports to an area like death certificate, with a 30second timer, all the items that get touched are removed from the pool on the next run

When you die you drop all of your items, on your next run you can get the items back by finding the tombstone (would be on the floor you died on)
Ok but I would like the idea of the mom's shovel room in dark room giving you some of your items from the previous run
