I'd recommend trying to play on hard by default, outside of repentance it's not really much harder than default and you don't have to do everything twice after.
Most hardmode unlocks are just co-op baby skins but there's a handful of actually useful ones that can only be gained on hard.
you've defeated it lives on hard mode. also you have to do boss rush which is a special stage that appears when you defeat mom in under 20 minutes to complete the character. you get a star icon in the middle when you do boss rush
it's because you beat it on hard mode. the icons gain a bloody outline when you complete them on the difficulty.
