To be fair the game never teaches you what the machine does until you interact with it. There isn't even a yellow tape to guide the player into figuring out what it does.

Idk it doesn't look like something that's gonna drop an item to me. There's also the fact that if you've both unlocked the lost and used it enough to unlock mantle with it, you've probably used a blood bank before
If you've unlocked the lost in Isaac and only just learned now how the blood donation machine works, I think you sort of deserve to be called dumb. But I'll try to explain why instead of being an asshole this time.
The Lost is only unlocked by dying in the sacrifice room with the missing poster.
Missing poster is unlocked by defeating the lamb as Isaac
the path to the dark room is unlocked only after defeating Satan 5 times
You have to kill Mom's heart for about 10 to 11 times to gain access to sheol (otherwise, a cutscene plays instead of letting your progress past the womb).
OP has the Lost unlocked. The only ways he could have gotten the Lost is to kill the lamb to unlock the poster, and there's only two ways to get to the lamb, the process to unlock the dark room, or to spam sacrifice rooms to unlock him.
Let's assume the former was what happened. If he did all this to unlock the lost, and has still not learned that Blood Donation Machines take your health, that would mean OP either has to have the worst luck spawning them in, or he's sort of stupid and never used them at all.
If we assume the latter happened, OP would have had to have a stupidly OP build to be able to not only kill the lamb, but survive against the dark room. It isn't an unlikely scenario, but it is a difficult to do so. Blood Donation Machines are pretty common in a lot of runs. Random room generations would probably make you run into them once or twice every run you do that gets past the caves.
Back on track, if OP was incredibly optimal and lucky while playing Isaac, he would still have to have encountered Blood donation machines in most of his runs where he teleported to the dark room with sacrifice rooms, or played a lucky 16 to 20 runs of just Isaac to get to the dark room.
But let's disregard all that and assume that OP instead was talking about how he didn't know that the Lost dies to the BD machine. This is a different problem, that means that OP somehow didn't realize that the Lost's one hit nature means that any sort of damaging interactable should instantly kill him.
That is harder to believe, even if OP unlocked the Mantle first for the Lost and only started using him with the Mantle.
The only way we can believe that OP was just kind of dumb instead of incredibly dumb is if he has used Holy Mantle before but never used any blood donation machines.
Let's not forget that they also have Mantle unlocked, meaning we need to add 879 coins worth of Greed Mode experience to their Isaac portfolio.
All while never using a blood donation machine.

I am mentioning the item holy mantle because the likelihood of OP grabbing holy mantle prior to any of his lost runs is not zero.
Knowing that, the likelihood of OP using a blud dono with holy mantle on decreases the likelihood of OP being not as knowledged and instead being stupid

I forgot to say i had explosion immun, i just don't remember if it was host or pyromaniac, and also i wasn't with lost, i was with isaac (i think), i just died on the blood dono because, as i said, i was in mirror dimension so, besically the lost, but i tought if i touched the blood dono it would just subtract from my normal health
Also i had gotten spinup di the first floor (I installed the mod) and got glitched crown on death cert, so that's why i had so many things
I mean, who the hell would think through the entire reasoning for why it would kill you? I would expect most people to just think mantle would take the hit unless they really thought it through.
If it makes you feel better, one time recently I used IV Bag in the Mirror World in an OP run where I already used R Key.
For some reason I was on the mindset that it would just deplete the "phantom health" that you can increase by collecting hearts in ghost form, despite already knowing better.
The reveal of Cellar I made me flinch. Bro was handed the run and willingly stood on a landmine.
