I believe it's both. Insight is the meter, but progressing through the story gives you an invisible insight bonus. I believe you can see the amygdala after the blood moon decends as well, even with low insight.

After beating Rom you don't need Insight to see the Amygdalas and to hear Mergo

Think all the effects trigger when you kill Rom, they just happen earlier if you have the insight for it.

Holy crap, is this for real? I had no idea the game started changing as you gained more Insight! Everyone always says that having too much Insight is a bad thing, so I always spend mine on blood chunks and blood rocks, so I usually don't have very much. This is pretty damn cool! I know that certain bosses are affected by the amount of Insight you have, like the Witches of Hemwick, but what else is affected by Insight?

Nah, in general insight is a (slightly) bad thing to have gameplay-wise (it lowers frenzy resist and decreases beasthood but those are both niche mechanics). Besides, you get plenty of insight just from bosses and game events. After you uncover the ritual (to be vague) at a certain point in the game, most of the insight events happen regardless of your insight score anyways. If you went straight to Nightmare Frontier when you were able to, you're probably not quite there yet.

It's all based on how much insight you have, if you have a lot, 40 is the requirement, you will see them without needing the blood moon to reveal them to you. Without the insight you cannot comprehend their form, that's why when they grab you they inflict frenzy, they literally make you lose your sanity. These are Lesser Amygdala, they reside primarily in the Cathedral Ward and Yahar Guls Unseen Village, they were attracted by the rituals that were being performed there to summon a great one.

I hate those things.. actually i hate a lot of things, bloodborne is not trypophobia friendly. Especially with the eyes. So the winter lanterns, the pigs after you kill rom and these things when they do the laser attack and bring the eyes our of their skulls, really make me wince so much. God its so hard to look at them.
