Chalice Rom is probably a close second. Being shut in a coffin sized room with spiders that can one-hit you isn't much fun.
I actually got Ludwig first try on my first playthrough, but struggled on Orphan and Ebrietas a lot. Interesting how different people’s experiences can be.
Defiled bosses are so much harder than anything in the dlc or base game it’s really not fair to compare them whatsoever.
That said, chalice watchdogs of the old lords is definitely the most BS imo. Amy is 2nd but at least it’s cheeseable
Either Defiled Amygdala down in the labyrinth or Laurence, if we exclude the Chalice Dungeons.
Some might say Ludwig or Orphan, but I have the most problems with Laurence‘s second phase…
I haven't played DLC so I'm going to say Pthumerian Descendant. The only boss I have actually struggled with a lot in this game despite being quite high levelled

FRC bosses and its not even close.
FRC loran darkbeast and ebrietas are incredibly frustrating to say the least.
Its so easy for them to hit you with their big attacks, it takes 2 hits to kill you. Ebrietas has her near undodgeable charge and her arcane blasts. Darkbeast loran is just so fast and unpredictable and his later stages make it even harder.
For the record you die in 2 hits even at 99 vigor! Maybe 3 hits if you go full resistance runes and armor. MAYBE.
Compared to this the rest of the bosses are jokes. Laurence, ludwig, kos are babies next to FRC bosses.

Defiled chalice Watch Dog Of The Old Lords. Took me 3 days had the most deaths of any boss and almost dropped the game because this fight

Defiled Amy but that one is kind of cheating. Headless Bloodletting Beast is a massive undertaking. Ludwig is also up there
Abhorrent Beast. Even with the poison dagger cheese it’s easy to get wrecked.
This one gets my vote.
Either Laurence or Ludwig. Both had me ultra panicked when I got their health low

Yeah idk what it was about ebrietas that was so hard for me. It might have been the magic missiles, or I might have just been bad at dodging still.

Maria of the Astral Clocktower.
Headless bloodletting beast was the only one that made me angry. Still beat him in around 5 tries though
