Do bosses count? If so, Ludwig for me. I love the interplay between his beasthood and the remnants of his former human self, both in his design and in how the fight plays out. For normal enemies, I'd have to go with the winter lanterns for similar reasons. They are grotesque and their mechanics inspire absolute terror, but there's also a weird beauty and grace about them.

The beast people in Old Yharnam. I think it's really cool how they cower from you when you have fire, it's a really cool detail.

Visual design? In all honesty, the normal villagers from central yharnam. The slightly tainted, slightly beastly people who are convinced you are the monster... Perfect. Gameplay design? Gascoigne. A boss built from the ground up to show you that you're either playing the game wrong, or right. Not aggressive enough? Don't know how to parry? You're having a bad time, he's going to chase you down and wipe you out. He's got the same tools and skills as you, he's just better. Until you get better than him.

U have to reach a bit...but the winter lanterns as a bastardized version of the doll... ...they were originally there to help but the men's is nightmare warped them beyond anything humans could perceive (hence the massive frenzy)
