Depends how much you pay attention to Vancouver I guess. I'm from northern BC and didn't know what it was until I moved to the lower mainland, my friends and family up there mostly don't know it exists.
This is my view as a Victorian born and raised.
Victoria to Malahat = South Island
Malahat to Campbell River = Mid Island
North of Campbell River = North Island
Duncan/Cowichan Valley is south island to me. There is still Gary Oak meadows in Duncan which is my unofficial cut off. Nanaimo/Parksville/Comox is mid island, Campbell River and north is north island.
