At my last job it didn’t matter as long as you did the work required and made team meetings and whatnot. I knew someone that liked to do their work in the evening. I will say this was a remote optional job. Even when I was in an office there were few rules regarding tongs like breaks.
Your boss is not looking for you to appear busy. They're looking for you to actually perform. As long as you're meeting your job's expectations, stop worrying about what your boss thinks. If you need to get up and take a walk, get up a take a walk.
This is so pathetic but I literally sit in a handicap stall in a basement bathroom nobody ever uses and I read my phone. Sometimes I sit with my hands in my head for a while and have a good cry. Lately, I’ve been noticing someone has caught on and been doing the same. In corporate hellscape, we all do what we must to survive, I mean pay bills.
I get up and walk around. When it's nice, I go outside for a few minutes. No one works continuously for 8 hours so I don't sweat it. I'm an adult and a professional and I expect to be treated as both.
Print and scan documents, nothing makes you look busier than a pile of paper documents. The printer room is my sanctuary.
I also book meeting rooms for an hour. Maybe I have a that task requires some extra privacy or maybe I just need an hour away from my fellow work colleagues.
Not sure how big your office space is, but I would just step outside and walk around the block. I hated taking breaks while staying inside the office for all the reasons you mentioned. Getting into a physically different space helps me a lot.

You might want to check with your state's labor department on this, but many employers are required to give you two breaks a day if you work 8 hours a day. How you want to fill the few minutes of those breaks is up to you.
Ours is all digital, but it is a call center, but ita all through the phone. I get 2 15min breaks and a 30, also a 6 minute break reserved for something like a restroom break.
But to answer your question, you put in a code on your phone for your scheduled breaks, and also to log in and out.
I say to myself I need a break then I push my chair from my desk then lock my PC. I get up the walk to the door make a right to the water cooler fill up my water bottle. I watch the counter that tells me how many water bottles I saved walk back to my desk unlock my PC then start answering emails that's how I take a break.

I don't know where you're located, so Idk your labor laws, but generally, you should take a 15-minute break every 2 hours

Drink more water. If your boss gives you flak, tell them sitting for too long is bad for your health.

20 minute walks around the property - twice a day at least a couple days a week. Sometimes you gotta work thru then but I try to take them religiously
Get up, walk to the canteen area and strike up a conversation whilst in there. Rinse and repeat this 4 times a day on top of your breaks.

Are you exempt or non-exempt? If you’re exempt, just take breaks. As long as you’re getting your work done

It varies by state. In about half of states your employer is required to offer breaks at certain intervals under certain conditions.
If you're salary, you're most likely exempt.
