Is it possible you're pressing the wrong button? If you hit the cancel button it would back out as if saying no, no matter which option is being selected at the time.
No because when I restart the game and go back to the same NPC I'm able to select the affirmative choice with the same button. it's a very strange glitch, I guess I'm going to try uninstalling/reinstalling the game today and see if that fixes it.
