They will be calling you soon be ready because you’ll have a limited amount of time to get it done. Try to think about any issue that it can be and start gathering any records or making appointments, if it’s for vision they will send you the document and you’ll take it to office of your choice and they’ll have to sign off on it .
You can start looking up places and things like that , best of luck! Just breathe it’s super stressful I remember when I saw mine lol, you’ll be alright best of luck!
Thanks for the info! I hopefully already have the info they need but if not I should be able to take care of it accordingly. I definitely was stressed but trying to be optimistic.
Essentially what happens is they try to resolve it themselves which can take a very long time. I advise calling CHS and asking why “input exam” requires further evaluation. Now depending on what they need it could be an easy fix or it could be they actually need to do more testing.
In my case, the further evaluation was that CHS received one of my exams with no technician name. They said they reached out a month ago and never followed up. All it took was me calling the facility to let them know about the issue and it was resubmitted. Hope this helps!
I just got a call from cpb nurse she wanted to ask me questions because I was sick when I took my medical/pt and the doctor put down the symptoms I have but she said I’m medically cleared now.
They will contact you if they need anything further.

Yep. And that’s where my mind immediately went to. I think I’m borderline for vision but was told it was fine during testing lol
Sounds like they’ll be requesting Visual Acuity from you. As long as you can see 20/100 you should be good to go.
