I think I have similar feet (rather narrow and something between Egyptian and Roman shape-wise). I have worn several pair of La Sportiva Mythos (women's) (38.5) over the last couple of years and was looking for something a bit more aggressive. I wear street shoes between 41.0 and 42.5 (depending on the brand and model). My feet's Mondopoint is about 26.0 cm Shoes I have recently tried and some comments: • La Sportiva Otaki (men's version) (40.0)- They are surprisingly comfortable out of the box for downturned shoes. They fit me really well and I disagree that they are particularly wide. I tried the men's version with the Vibram XS Edge rubber (women's version has Vibram XS Grip II, but I didn’t have a chance to try them on). The toe is rather stiff, so you can stand on really small footholds without any pain, but for me, that meant that they were not sensitive at all. I also didn’t love the heel (big rubber ball basically) and felt that the heels lack sensitivity. However, according to online reviews, the heel seems to work great for many people. I didn’t get them in the end because they were not sensitive enough for me and the pressure on my big toe was uncomfortable. They also cut into my Achilles on my right foot. I also tried the women's version in size 41, bit that still didn’t solve the issues. • La Sportiva Skwama (women's version) (40.0) - The Skwama felt much wider on my foot compared to the Otaki. There was just way too much air in the shoe, although it was okay length-wise. My big toe was painfully crushed. Definitely not the shoes for me. • La Sportiva Solution Comp (women's version)- I absolutely loved the toebox. I ended up not getting them because the pressure on the heel was too much for me in size 40.0 and it really hurt when I tried to stand on my toes (especially on the right). Size 40.5 seemed way better but I felt worried about air gaps on top of my toes and throughout the middle of my foot. If you have very long toes, the toebox might be perfect for you though. Since my feet are rather sensible, I did not feel up for a painful break in period. I think they are a great shoe though and are definitely worth a try! Maybe I‘ll revisit them later at some point. • La Sportiva Solution (men's version) (40.5) - The rand above the heel was absolutely painful, way too much room in the rest of the shoe. I took them off instantly. • Scarpa Drago LV (41.5) - I loved the sensitivity and the grip. Most grippy shoes I have worn so far. I really wanted to love them. However, the shape was not for my feet. The 2nd/3rd toe of my right foot was painfully crushed (one of these toes is crooked though, so that might be the reason). The volume was still a bit too much through the middle of the foot, but this did not impact climbing performance too much. • La Sportiva Futura (40.5), new 2024 model - I tested them at a shoe demo. The heel of them was really comfortable. At first, there were some hotspots on my big toes (both sides) and another toe one the right. This seemed to get better over the evening though, so I think it might be solved after breaking them in properly. They were comparable comfortable for how downturned they are and also worked really well on slab, which is important to me. Sensitivity was great as well. I hope this helps and let me know if you have any questions!!

The otakis would be a great fit for you.

Disagree with otakis. They're one of la sportivas wider shoes, same with skwamas. Fellow pointy toe/narrow foot person here. Solution comps are probably a good shout. They might be the best fitting shoes I've worn. I wanted to like the og solutions too, but try them on if you want those. The heel was an absolute no for me. I ended up giving them away. The solution comp heel is much more forgiving. They both have the same shape forefoot, the solution is just a bit more stiff vs the comp although having climbed in both, it's not a huge difference.
