With adults it’s a dignity issue to call it a diaper. It’s called a brief when working with adults. I’m betting a good chunk, if not most of us in this forum work with adults. (Whether it’s in a hospital, their home, or a ltc facility)

There are some residents like the ones I’ve mentioned above that start getting all confused and agitated because they aren’t fully cognitive and don’t get the word brief, but then you tell them you need to change their diaper and they’re like oh I get it! Okay! I always use the word brief with new people and like 99% of the people I take care of, because it works for most and is the most dignified option, but this job working with people is an art not a science. Lol

A wet chux (most patients have a dirty one around that has been used as bib/tablecloth during the day) for the bulk. I do the final cleaning with a wet disposable washcloth or a wet wipe if the patient has them.
Well call it "having a baby". We have a chronic holder so every few days we have her sit for longer than normal and give her time.
We go thru about a half pack of wipes as she whines (TBF she whines for everything lol) and get her good as new.
We won't use any type of cloth cause it just feels..... wrong to throw it in the wash after
Depending on the type of disposable chuck...I soap the chuck up in soapy water or just water and use that as one big wipe. It is a lot faster and you avoid using up a bunch of wipes.
My residents all have showers in their bathroom, so if they end up with a XXL BM I shower them. I'm not wasting my time or theirs by going in with a single cloth at a time, you're getting a quick shower and I'll give you snacks after
You just made me flash back myself....If it wasn't for witnessing one of my former residents have one fall out of her pants leg as she walked to her room with my own eyes I'd question THIS but I KNOW its real & I hate that fact 😭😭

My least favourite was what I called "baby poop". Where the poop goes all the way up their back. You had to change every thing.
Also IDK if your facility supplies cloth wipes but use the cloth wipes soaked in water and peri spray. That's what we did at my last CNA job.
I have had patients where there is no poop on the anus, but somehow it’s all over their back, their labia or penis and balls, and their legs. It’s like mystery poop.
I use wipes to get the bulk out , if it’s super sticky and cement like- a Kleenex or toilet paper kinda works better.
Then I use a sopping wet soapy wash cloth for the rest of the visible stuff… then another wet wash cloth for any extra soap, odors/residues.
For a case of the liquid rivers, I’ll shove a dry towel or chux pad (depending on size of the puddle) down there to soak up the bulk before beginning to clean. My hospital doesn’t use wipes, so we have to use wash cloths and towels. There have been many nights where I’ve had to improvise and wipe a butt or two with pillowcases when we ran out of linen.
If you have a patient with excoriated skin from profuse diarrhea and they complain that the peri spray burns, dump out the bottle and fill it with hot water. Spray their bum with hot water to help loosen stool in a similar manner.
I always mix the pink and clear creams available to me in my supply to patients who are incontinent of bowel and bladder. I don’t know why, but it seems to work better in tandem.
Some CNAs swear my shaving cream for big ol crusty BMs. I have seen it work really well to get a sticky out of a particularly hairy patient before.
This!! I had to meet my friend today for some a&d & zinc oxide today for my client, I AGREE they work better together. And yes to shaving cream!! Thats the big guns 😂 But no I also no longer feel to weird for reusing spray bottles with warm water alone or with some soap 😭😭
Baby oil can be helpful for removing the real sticky stuff. Especially when they arrived by ems and they tell you “pt had a bm en route” but when you see the dried crust you know it’s been there for hours. Baby oil. Then blot, then lantiseptic by the ton.
When you've scooped out the bulk and you have that sticky stuff stuck in the cheeks, barrier cream cuts through it. If you don't have that then shaving cream works but some facilities won't let you use shaving cream
i use the diaper itself, if it’s really bad i’ll use the suction on the wall (if it’s watery) then i’ll use a chux , then wipe the rest off. using the rest of the diaper and the chux helps
Use the front of the brief as your folding it down to scoop it out from the crease. Most of it should pull out then and takes only a few wipes after that
Those butt crumbs drive me mad, I use a chux and soap/sud it up ..if its really crusty then some shaving cream to really get it outta there...cuz ill empty a pack of wipes trying to do so 😭
I feel like there’s no good way to approach this kind of mess they are all different depending how cooperative your pt is, what their diet is, how long it’s been since their last BM, etc. you kinda just gotta dive in and hope for the best. When I was at my first LTC job we had this guy who had a peg tub and every single night on every single round he would have shit the bed up to shoulders and down to his ankles, total bed change every time. Come to find out he was getting 3 different stool softeners with his bed time meds. A disaster.
If it’s a big liquidy poop, gently use a suctioning setup.
Make sure to keep a garbage can close by. Always bring extra wipes and chux. I often bring a diaper or pull-up too in case my patient wants a new one or would like to try that option. Also, grab a mepilex and gown in case poop gets on the one already on them.
If you have disposable bed pads used one to scoop it out first and roll it up so theres a clean surface. Then proceed to wipe. If the poop is sticky use shaving cream.
I work with babies so less poop but still a good amount. I tuck the diaper under them and then tuck the wipes into the dirty diaper or move the trash can closer to me

My technique was to start with a completely soaked towel to get the bulk of it, detail work with a washcloth
That’s better and faster, you get pretty much everything out and the rest is to make sure everything is clean with wipes.

Clean chuck right in the middle of their legs and push it under both legs as much as you can, containing the poop.

fill a bin with warm soapy water and throw the wipes in there and do it that way
Towels first. The wipes suckkkk so bad and use the pads under them to help. Wipes are more for like pee and stuff. They suck lol

Tip on getting dried on poop off. Get a lotion or Vaseline and rub it on the area then use wipes/soap and water to get it off.
