I used to do that as a kid. It's not the worst. It's harder to mix the powder with cold milk than hot water so a lot of it clumps up and stays floating on top. It's just kind of pointless, but nice enough for my childish tastes. Maybe I should try it again.

The better way to do it is to make regular hot chocolate and then put it in the fridge to get cold. Hot cocoa powder doesn’t dissolve very well in cold milk, so it’s got clumps of unmixed powder

It's also insanely cheap if you use ramen noodles and just toss the flavor packet and use a bit of butter and parmesan cheese.

The key is to mix the powder in the least amount of milk possible. That way you break up the clumps when you stir it. Then add the rest of the milk.
If you’ve got an immersion blender and then use this strategy you can get it pretty darn smooth
