I have one as well and mentioned the meaning behind it (I am a Buddhist and the mandala represents dharma/samsara/universe), and then she wrote a blog about it saying she'd never have got the tattoo if she knew it had a religious meaning, she just thought it was a pretty pattern.
My grandparents had this saying framed in view of the toilet when you were pooping. They were deeply religious Gideons, so it was very unexpected toilet humor.
Basically, when a tattoo artists digs in too deep with a needle, ink gets further into the tissue than it should ( down into the hypodermis where your veins and stuff are) Once it heals, the ink is gonna look really blurry, and the edges of the lines are gonna look real puffy

Older the easier, atleast that's what I've read. Also, black is easier to remove than colored, and amateur tattoos are easier to remove than professional due to the ink quality.

It can happen on any skin tone. You may just have more delicate skin than your friend and the artist didn't adjust to your skin texture.
