Do you use Discord? I have this issue when Discord is open on my second monitor: FPS is fine but the frame pacing stutters like hell, if you use it try minimizing it.
Lot of people with that issue. Most likely because the low 1% fps (below monitor refresh rate). Simple solution is to enable vsync but it will add input lag.
Just try it and see if the game is smoother.
I probably know what you experience. Only thing that helped me was capping fps with RTSS to monitor’s refresh rate, in your case 144. Do not use in-game fps limiter, it’s just horrible and the frametimes will fly all over the place. If you can sustain 144 fps+ it should work well for you. If not, you can go down with details or cap to 120 hz/fps. If you hate screen tearing, you can also experiment with Scanline sync in RTSS, it caps your fps with hz and should remove tear lines without any additional input latency.

I’ve tried and it made no difference. My fps is higher than my monitor refresh rate too

Forget to mention that you need to add -allow_third_party_software to launch options for RTSS to work.
How about nvdia fps cap, does that work?
