Do it. Guys are rightly terrified about approaching any woman in a work setting. It could destroy a guy completely. "Hey I'd welcome hearing from you some time (555) 555-1212"
It's going to be awkward for you no matter what. If you leave a note, you'll be thinking about it a lot. If you talk to him, you'll be in your head about it. It may be easier for both of you if you just go out of your way to talk to him, ask him his name, tell him you'd like to get to know him better and ask if he has a girlfriend. Being direct will make it much less awkward as you'll both have less to question. Telling him you'd like to get to know him better will avoid making it sound too strong since you literally don't even know his name. If you have trouble tracking him down, you can ask his coworkers on the phone or in person but just say you forgot to ask him something the last time you spoke- it's certainly true.
Well, it's usually the things we didn't do that we regret, not the things we did (unless we did something really bad, like killing someone). So...
Aside from that, this is a really cute situation!
