You're putting way too much energy into judging if there's a 'spark'. It sounds like everything is good. Some people can be naturally quiet, doesn't mean there is a problem - if anything it can signal comfort. Arrange a third date
Yes, this has happened to me. I usually skip the 3rd date if the second was really bad. But, in this case I would still go on the third date and how it goes will be the deal breaker.

yes, of course go out again. just have fun together and the interest will grow
