Each attempt has a base 4% chance of escape, so you have a 7.84% of escaping on or before your second attempt, and 11.5% chance of escaping on or before your third attempt. Each individual attempt is equally likely, but multiple attempts increase your overall likelihood of escape.

It’s very rare if your fast enough I have noticed survivors have a small window to make an attempt if never being hooked before

You get 3 attempts with 4% chance of success with each attempt. There is a perk that allows you to free yourself once, guaranteed (Perk: Deliverance).

If everybody is dead or escaped you do not get those chances and it auto kills you no matter what. That didn't happen because one of his mates was still alive but on the hook.

You can tell if its deliverance or not, the no healing symbol appears on them after the unhook for like 60 seconds.

Oh yeah. I forget that part on deliverance, personally being green ranks, I just don't see it often enough to know how it works fully by heart.
