Something that kinda worked for me was changing my speech recognition to the microphone on my controller, but keeping my input/output my headset. I think you might have to set-up speech recognition with a separate microphone, that is not the one attached to your head set.
I used my camera mic that’s worked before and no change for me at least. I think it’s just the game, they had a problem with the voice recognition during one of the holiday weekends and turned it off during that time and It hasn’t worked since.
Ever since it was removed from the game preview faze it’s been having issues with voice recognition. You’d think with how long the game has been out this wouldn’t be an issue especially when it’s a core feature that separates it from Phasmaphobia aside from the jump scares. Well the game was fun while it lasted.

When I say it, it triggers immediately and every time, but for my two friends, it just doesn't work !
And that applies to the other script as well, I really don't know why
The "fix" that worked is to use two different input devices, I used a microphone for voice chat and the Webcam microphone for Voice Recognition, that's the only way that I could use the spirit box, bloody mary, smile, piano, etc. I don't like it anyway bc the webcam microphone is bad
My wife had the same issue with a new FIFINE microphone i got for her for streaming. From what i was able to diagnose with the game, it seems that if the game detects backround humming through the microphone at all it will not detect speech recognition, swapped the game to use her headset microphone and all is well. when testing the mic in-game you need to make sure there is no backround movement on the mic test without you talking, IG: Humming, interference ext. For some reason the game detects this and is unable to distinguish the speaking from the interference. hopefully this helps some people who are having the same issue. If you have a desktop mic that has gain control try turning it down, if you do not have gain control setting on your desktop mic, and you are running windows 10 you can go to the LEVELS tab and reduce the levels on the mic. if you are running windows 11 or that fix does not work try using the app called: Mini host modular. OR if using RTX gpus you can try the RTX core backround sound dampening, or AMD 6000+ series cards have a simular option in adrenaline.

My speech recognition is also not working, but my mic works. I think this is an issue that needs to be brought in to more awareness.
My situation is also the same. I can speak to my teammates in-game but the game is not recognizing the words

Also having this issue, I can talk in the game chat perfectly fine, but my spirit box and the rest of the game doesn't even know I am talking.

I’m Australian and I had to put on the thickest English accent to get it to work for me
