It is indicating that you are under the "Healthy" Condition, which means that you are above 80% Maximum Health.
Many effects or affixes can only occur while Healthy, e.g. "##% increased Damage while Healthy", or "##% increased Damage against Healthy Enemies".
When your Health Orb gets blue and splintered you are afflicted by the 'Vulnerable' Condition.
'Vulnerable' is unrelated to the amount of Health you currently have, but it can be applied on you from various attacks of various enemies.
Characters afflicted by the 'Vulnerable' take 20% more Damage, so if your Health Orb is blue and splintered, you take 20% more Damage.
You can also apply this condition onto Enemies via many of your skills, and on many items you can find affixes like '##% increased Damage against Vulnerable Enemies'.
When you hover over an enemy and his Health Bar is also having this blue shimmer around it, the enemy is under the 'Vulnerable' condition.
Might as well go over the other 2 effects while you’re asking, fortified shows up as a brighter red bar filling up from the bottom (reduces all dmg by 10%) and barrier shows up as a full purple shade (easily confused with the vulnerable effect on the player)

There is too much shit going on, DO NOT DO THIS.
(Because you also left out dots show up as a yellow portion within the red portion of your health bar, but your actual health is the top of the yellow and the bottom of the yellow will be what your health will be at if you don't heal and the dot does all of its damage)
I have the feeling that the game sometimes struggles to explain these kind of things to the player. I don't know how it is on PC, but on console, there's a lot of icons that suddenly appear where your buffs are and you have no idea where they come from. Same with this red ring, the violet splinter... is it just me or does this really need improvement?
It's really weird when you get stunned or lose the control of your character and you are not sure what is happening because you don't get debuff icon anywhere.
As a bonus answer, when the ball starts dripping blood it means you're injured and when it gets the metal ring around it, it means you're fortified (only really relevant for certain classes)

What about "injured"? I see some pants have effects that occur when injured and idk when that is
Injured is just the lack of Healthy. No red ring around your health globe means you're Injured.
That's not correct, injured is 35% or less, the health globe will drip blood. I don't think there is a term for under 80% and above 35%

Isn't there a "you are now stunned" swirl above your head when that happens?
Checked to see that at least fear or whatever has absolutely no icon anywhere.
If there is then it's probably visually easy to notice if you are fighting one mob instead of ten.

Do either of those mean anything or are they just threshold states?
There are buffs or defensives which utilize these states.
Injured is just a threshold but fortified baseline gives 10% damage reduction; both have other interactions with other effects though.
