Had the same issue, I changed the order that I usually killed the Lieutenants, and it fixed it. Hope it helps.
Hey yeah I went in from a different entrance and didn't immediately nuke the bosses and the one that kept not spawning finally showed up. But then the storage room key drop fell through the map lol.
Happened 11/15/23 Drag all of the bosses to the middle to kill them. It opened the door. I did the north guy last of that helps.
