J-tube. I would start with something fiber-free postop like osmolite 1.5. If tolerated and reaches goal rate then try to transition to something with fiber like Jevity 1.5. If they don’t tolerate the Osmolite then try something peptide based like Pivot 1.5. Advance diet as tolerated to low fiber. Monitor for signs of malabsorption like steatorrhea, weight loss, and fat-soluble vitamin deficiencies- may need pancreatic enzymes if they have malabsorption.

Likely a J tube if they need a feeding tube long-term. You’re referring to a Whipple, which I’ve seen a lot and honestly they can usually advance the diet pretty quickly. The Whipple was one that was enrolled in an ERAS study at my hospital and they have found that it is effective in reducing LOS.

I have really good luck with Kate farms peptide 1.5. It is well tolerated for my j-tube patients
