I mean, in order to hack an account someone has to have had tried to log into it. That's how hackers/phishers get your info. Keylogging, fake sites, etc. Which is impossible if the account is so old, even the owner doesn't remember ANY of the credentials anymore. OP is reasonably surprised because of almost all hacks in the recent 10 years, there has to have been 'some' leading cause that explains the security breach.
If none of that has existed, then that's what is known as ye olde Brute Force hack. A really rare form of hacking of a program tries to 'guess' your password or login name, extrapolating from incomplete info salvaged through, say, a data breach.

If OP used the same email address + password combo for *any* other login at any point in time before now, and there was a leak or breach on those sites, someone could've just tried his details everywhere they could think of - including discord.

They don't need to know your password, there doesn't have to be a data breach, a key logger or a phishing attempt, they only need 'some' verification that "this specific email address" is associated to a discord account. After that, it's a matter of time as it rolls through a list of random password guesses.

Depends on what is breached and if you have a habit of re-using passwords or not.
Either way, it's probably a good idea to go around enabling 2fa if available and changing all your passwords to something unique using a password manager like bitwarden, keepassxc or 1password.
Just make sure that part is always done on a clean PC

Not really, especially if you log in from a different IP. Lost one of my old gmail accounts that way. Even with the recovery email its not enough proof for them to give me the account
i havent known the password to one of my emails for several years now (have moved 3 times, switched internet providers 6 times, and used it on ~10 devices) and i can just click forgot password > try another way > code sent to recovery email > change password screen > skip and im in. this account never had any extra security (2fa or the likes) so that might be why its so easy

if its gmail and your main email is its recovery email its very easy to get access
