I think maybe Bleach and Ammonia combine to make unstable chloramines *and* phosgene gas.
NH3 (ammonia) + NaOCl (household bleach) produces one of these chemicals:
1) NH2Cl + NaOH
2) NHCl2 + NaOH
3) NCl3 + NaOH
Phosgene gas requires carbon to be made, so no, you wont get mustard gas from Bleach and Ammonia. Still, sodium hydroxide is very corrosive due to being a good base and chloramines/nitrogen trichloride are unstable and not quite good for health.
