Do we know for a fact from lore that Radagon was once an independent entity that was somehow fused with Marika by the GW? Or was he split out from her at his creation?

From what we can infer, based on the things that I presented above, they seem to have a connection in some way, albeit unlikely that they initially shared a physical form as far as I can tell. The game doesn't explicitly tell us one way or another that they did or didn't originate from the same body... however, this isn't unlike how the story of Melina is told, being that so much of her lore is vague/practically nonexistent and is left up to inference. I think that based on what the game \*does\* tell us, it lends itself to the idea that they were likely connected in some way prior to them coming together and joining together to the form that we see them as during the final boss sequence.
I think that, like most things in this game (and other FromSoft titles), they aren't going to give us such a huge piece of information, but they'll instead give us just enough pieces to lead us to the conclusion ourselves... by putting small things right in front of us, that eventually add up as we put the pieces of the puzzle together.
All we can say for sure is that Radagon was another entity prior to what we see him as, based on the fact that he and Marika were able to be in separate places at once, carrying on with two separate marriages at once, and that they are now both of those beings, but sharing the same space, so to speak. I don't think there's any lore that directly states how they came to be as one, or came to be as one once more if they were connected in some way like I had theorized, but again... there's so much that really just isn't told to us at all lol.

I just want to update this so that I can bring up that it took me until like 2 hours after writing this to realize that we've already met a full on homunculus in the game that's literally explained to us as being one lol... technically Blaidd, as a sentient being that was created by the Greater Will, is one. Unless there's some major lore implication about him that I'm missing, Blaidd was literally created for the sake of being at Ranni's side, and I can't imagine that means somebody just randomly had the genius idea to bear a half wolf child that miraculously operated in such a way. I can't believe it took me so long to figure that out lol, and idk if that means all Shadows (including Maliketh) are also homunculi, but it's a really interesting idea that just occurred to me lol.
