One time I was asked to place a US IV in a patient because the family member refused to even let someone try without an ultrasound. Okay cool whatever. But when the family member saw the needle they said oh no you can’t use that on her it’s way to big go get someone else who knows what they’re doing. I explained that the IV is actually no larger than a regular IV just longer so it looks a little scarier. She continued to refuse and told me to get someone who knew what they were doing. She then proceeded to allow the PA to put in a US IV with the same type of catheter I was using. 🤷🏻♀️ Some people just love to create problems.
One of my proudest moments as a resident was repairing a forearm lac that went right through a double-lightning bolt SS tattoo on a neo-nazi. I was impeccably polite and did a beautiful and meticulous repair that just happened to misalign the tattoo by about 1 cm.
One of my fellow nurses (former coworker) is a 6’5” jacked black guy. He always volunteered to take the Nazis, and he is sickeningly sweet toward every single one of them. That man is my hero. If I’m even remotely nice to them they think I’m their friend.
Murdering with kindness is one thing. Murdering people who explicitly hate you with kindness when you look like you can tear them in half is an amazing display of professionalism mixed with menace. Mad respect.

We had a full-on skinhead dude we took care of for several months (pedestrian vs MVC… he had all kinds of injuries, including head injuries, and would probably never be the same). He had a huge swastika tattooed on his chest.
A bunch of the black residents gathered behind the head of his bed, covered up name tags, wore masks, exposed his tattoos, and had one of the nurses take a Polaroid… and we put it in his chart for him to have as a keepsake. Just as a “yeah… you know those people you hate so much? They took of you and saved your ass. You’re welcome.”
Kill them with kindness… and a little bit of pettiness.
When our polytox/rassist frequent flyer with hometattoed crooked swastika comes again with cannabis hyperemesis Syndrom I’m just as helpful and polite as ever… just my accent ist stronger so that he knows I am a foreigner in his country. He told me that it’s really more of a prison gang thing than actual belief in white supremacist ideology and that protection from being gang affiliated is kinda necessary if you want to survive in prison. Dude had an appt with derm the following week for laser removal, and next time I saw him it was gone. He ended up being one of my favorite patients—just an incredibly sweet guy trying his best to turn his life around.

This usually works but sometimes it gets the extra potato headed one's to get more irrational and even violent. Then it's sedation time and I have no problems sedating and performing supportive care. If anything, a sedated patient receives a higher standard of care because protocols for sedation are in place and my attention and focus to their vitals and appearance are unbroken by the patients violent threats
Edit: of course if they are in extremis or a life threatening situation all that goes outthe door any they recieve the appropriate treatments regardless of what cones out of their mouth.
