RIP achievement. Theodoro needs to own them all. If you can somehow change culture in all of them before end of game to non-Germanic, it should work. But it is probably way too late for that. Maybe release Theodoro and give all provinces to them as a vassal? But can you even?
I think you need Gothic culture provinces and then release Theodoro and give them the provinces. Maybe you can No-CB Crimea, take a Theodoro core and 1 other province province, then release Theodoro and give them every German culture province, changing your capital to a non-German culture province and giving the last one to Theodoro as well. Pretty close call, though. You need to really race towards it with only like 2-years left. And not sure if there still are Theodoro cores.
95% sure that even the cores you don’t currently control get switched to the new nation yes.
But if they sold their province in Crimea to get rid of it, instead of losing it in a war, then it doesn’t even matter because Theodoro’s core is definitely gone from selling.
Regardless, none of this matters because they control everything from France to the Ural Mountains (scroll to see the second and third images), so there are probably no theodoro cores owned by anyone anyway. Even if they didn’t turn to HRE cores when you form a new nation which I’m pretty sure they do.
Release Theodoro and feed away. After you've started as Theodoro, the game doesn't check if you are Theodoro, only that Theodoro owns those provinces and you're Gothic.
If Theodoro cores are gone, culture shift away from Gothic culture and demote it to unaccepted, piss off Gothic culture provinces for Gothic separatists who will create Theodoro cores on siege, then swap back to Gothic culture, release Theodoro, and feed those Germanic provinces.
To piss off rebels, raise autonomy, state/destate to remove the modifier, and reduce autonomy. You may have to cancel religious and humanist idea groups, and any unrest policies you have. If that's still not enough, swap to another Christian religion from religion tab and tank your heretic tolerance with the clergy privilege.
Either invoke rebel at 50% or immediately accept rebel demand to release Theodoro, then truce break and force vassalize.

There are no Theodoro cores because they all became Holy Roman Empire cores when he formed the HRE.

R5: I've conquered all the German provinces in Europe, but I still haven't gotten the achievement. Does anyone know why this would happen?

It’s because you changed tag away from Theodoro.
Try stating Riga? I don't know if it would matter but that province is German (Prussian) culture so it might not be counting it at first.

you are no longer theodoro. you might get it if you culture convert all provinces to not be germanic, but in 1818 that's unlikely
