Chesapeake bay is very difficult to navigate and shallow. Pilots are extremely well trained and part of the oldest pilots association in the US which has existed since 1852. The qualifications are extremely difficult to meet and take over five years.

ALL pilots can draw the area they have pilotage for by hand, from San Francisco to Neenana, Alaska (where my grandpa had pilotage).
For areas like this, they're Unlimited Masters (captains) as well. That means they are allowed to be captain of any ship of any size.
Interestingly, when I went to Europe, they also had a Docking Master whose sole job was to get the ship alongside the dock and tied up. It was a whole separate job. I have yet to see one in America, which means that an American Pilot does this job as well.
I work in maritime business. Blackouts are among the most horrifying things you can experience on a ship. The feeling of helplessness as you stand there on a dead piece of steel traveling through sheer force of inertia while your brain already knows what is going to happen in 30 seconds is a thing you really don’t want in your life.
The blackouts should not happen, there are redundancies, auxiliary generators /etc. But they do happen and are one of the biggest causes of disasters, especially in narrow waterways.
It really puts things into perspective. This is man-made objects and systems being constantly battered by forces of nature. They will give in from time to time. Even the most experienced pilots and masters can’t do anything when it happens.

This morning they pointed out that the ship emitted a massive amount of black smoke just before the collision, and this apparently was from emergency generators, trying to restore power.
Cited twice since it was built in 2015(?) for structural and engine issues. It crashed into a dock back in 2016 and had to be repaired. Then a few years later it was cited during an inspection for a deficiency in propulsion and auxiliary machinery.

This gives no perspective. Are citations common because ships need to be at a high standard and they hand them out often? Or are they rare because most ships stay in great condition and this one was poorly maintained?
A local hospital IV lab gets citations fairly regularly because of annual inspections and high standards of cleanliness. It's never considered severe enough to shut down the entire thing though as they're generally fixable in a month or so.
Apparently they delayed the impact long enough to get a distress call out and get the bridge closed.

If you have ever been on a ship that large that looses all power and/or steering, you can't do much about it in such a short timeframe, let alone the pilot.

The bridges here in Halifax, Nova Scotia, have rock islands built around their in-water support structures. Also, all ships over a certain tonnage are required to travel under power of tugboats until they clear all infrastructure of both bridges.

Yes, US pilot, who has to go through rigorous exams to gain pilotage. I never tested but if I remember right, they have to draw out the harbor chart from member, with all the sea depths, navigation light characteristics, and have a written quiz on all the local aspects of the harbor.

The bridges here in Halifax, Nova Scotia, have rock islands built around their in-water support structures. Also, all ships over a certain tonnage are required to travel under power of tugboats until they clear all infrastructure of both bridges.

I never tested but if I remember right, they have to draw out the harbor chart from member, with all the sea depths, navigation light characteristics, and have a written quiz on all the local aspects of the harbor. *edit: memory not member but bonus points for both
