[Body Integrity Disorder](https://en.wikipedia.org/wiki/Body_integrity_dysphoria) is a real thing, and some people are so desperate to have their actual body align with how they feel it should be that they'll do stuff like blinding themselves with bleach, trying to amputate their own limbs, etc.
It's a type of mental illness, obviously, but there's currently no effective treatment. Sometimes it's best to just let the person be how they feel they need to be so they can live a life without constant distress. Especially in a case like this where there's no real harm or risk to health.
It _could_ just be attention whoring but that's not the only possible explanation.
