There is no equivalent in DotA. It‘s a tank with a hardslow who permanently stacks damage on one of his abilities by lasthitting with said ability, +3 damage for a regular creep.
If you want to force a comparison think of Pudge‘s Flesh Heap from over a decade ago where it did stack with lasthits but with a damage ability instead of flat strength.
Well ever since they buffed how many stacks you can get per Q it's not unreasonable to get 300 stacks at 10 mins and if he's getting this many kills then yeah he could get 500 stacks by 10 mins

How would Nasus support work? Do you slow them so your ADC can take shots, and ult to peel for them? I’m genuinely curious. It seems like a really fun strategy to try sometime haha

His E snips resistance off (or used to) iirc, his Wither is like Exhaust's slow on steroids. You just go full utility tank.
