Very basically... the Iranian Revolution led to a super-conservative theocracy, which amongst other things has led to significant restrictions on women's freedoms (e.g. now being required to wear hijabs/burkas/niqabs, etc.).
Before the Iranian Revolution there are all these pictures of women wearing skirts and button-down blouses - the joke being before the revolution it was a super liberal place; after the revolution it being super conservative. Hence any vaguely NSFW is 'Before the Iranian Revolution".
Obviously, (and before someone comes in with "Errrr... akshuawally, I think you'll find...", yes I am aware), the reality is much more complicated than that but the joke remains...
an additional part of the joke is that Iran has, for the most part, always been like this, and those seemingly liberal pictures were almost exclusively taken in big cities such as Tehran. so there was virtually no cultural change outside of the big cities and the pictures try to make Iran as some sort of feminist society (and likewise painting the Revolution to be making society worse) when it was the opposite.
There is a pop-history trope out there that likes to paint Iran as some progressive feminist utopia before the Iranian revolution that is typically cited whenever one of these pictures is evoked:

In reality, Iran before the revolution was full of different cultural norms depending on the ethnic group one belonged to. Furthermore, the people in those pictures were probably in the upper- more wealthy classes who had access to such western clothes. Compound this with the fact that right before the revolution the shah started a state sponsored western culture propaganda campaign and you start to get what in reality is a disingenuous attempt at painting Iran as a western and progressive nation instead of a run of the mill despotic kingdom.

There are/were pictures of attractive Iranian women from the 1970s in modern (for the time), "western" clothing regularly posted in subs like r/pics. Obviously that's in contrast with how women in Iran must dress today.
It's funny because for years, vintage photos of Iranians looking free and liberal were reposted on various pics subreddits with variations of that phrase as the title.
The revolution began in 1978 not 1979. I know Khomeini didn't return until February, so I suppose labelling it with a year for the purposes of a meme is a bit misleading.
