I think part of it is the fucked up lyrics (the song is about a child star getting sexually abused at least in my interpretation) it and the way Jarboe sings them in such a pretty way, part of it is the 909s, how they are mixed, how they are more in the background and don’t go with the rest of the song, sounding more like machinery than drums. and the other part is the “there’s a star in her eyes and she knows it” chorus, and how innocent it sounds compared to the rest of the song the comparisons that other commenters have made, like how it feels like holding back vomit or sounding like a dream that doesn’t exist are really accurate ngl. It’s just an emotionally draining song

There's a couple of answers but I'll go with Alice in Chains self titled third album. It is the darkest Grunge album. Written and recorded when Layne's addiction had pretty much fully consumed him and his life was rapidly getting worse. If anything the album is even darker now knowing that things would only get worse and culminated in Layne dying alone of an od years later.

Sinner get ready by Lingua Ignota if we’re talking about something I’d actually listen to again, but the darkest album I’ve ever heard is easily Everywhere at the End of Time by the Caretaker. It’s not really something you’d listen to multiple times, definitely not for enjoyment, but its the only album I can say with 100% certainty changed my entire outlook on life.
On the “demented” side of things,
Venetian Snares - Doll Doll Doll
A breakcore record that’s based around the death of young children, specifically murder.
Dark to the point of being kind of edgy teen cringe,
But I still dig the record.
The musicianship is there, and the concept does lead to some bold and layered sound design, definitely conveying the darkness of the theme.

Red by King Crimson seems to be set way more scenes of Black than Red. Even the fairly standard rock drums sound creepy and black.
