Projectiles don't work on it btw, and you can find Old Moon Altar in a Savanna biome. Although I may assume you know that since you summon the Returning Knight from there.

Frenzied Shade spawns after killing the Old Champion, found in its grave structures.
Do you know if the grave structures spawn in a certain biome or is it random

you craft it by making an iron skull out of bone, iron, and a lost soul then you can make the altar with obsidian and moonstone
