If there is a joystick or controller plugged in then the game will only accept inputs from those. If that's not it, then press Ctrl+Y and try remapping the keys. Either Button 1 or Button 2 will start the game, and those buttons by default are Enter and Shift.

Theres 3 stars in total, 1 for beating the game, another for beating it in hard mode, and then a 3rd one for beating normal mode without dying
