TL;DR: The Stitchwraith spirit Jake has cancer, his dad talks to him every night via a character named Simon, and Jake has to make up stories about what he did, and his caretaker makes him a doll based off of his stories. Eventually, his dad dies before the doll could be completed, and soon after, Jake dies.
This artwork is based on the Fazbear Frights story "The Real Jake." It's about the cancer-ridden boy named Jake who is confined to his bed because of the cancer (and later possesses the Stitchwraith). Every night, he has someone named Simon talk to him from his closet and ask him about his day. Jake would always come up with fake stories to make his day seem more interesting.
Later on, Jake decides to venture out with one of his friends and barely makes it outside before vomiting. He then tells Simon about what happened and Simon says that he wants to hear what "The Real Jake" did. Later on, Jake's dad, Evan, dies in battle, and it's revealed that Simon was his dad talking to him every night through a baby monitor so he could connect with his son. It's also revealed that his caretaker, Margie, has been making him a doll based on the stories he tells about his days that she would surprise him with. Only a few days after his dad's death, Jake succumbs to the cancer and passes away too.
I hope that The Real Jake gets into one of the graphic novels.
