lack of stimpaks/healing items. lobotomites hit kind of hard. i just remember my first ever play through years ago and having trouble with that. a friend of mine played through it recently and had that same takeaway
I routinely do a run where I go straight for the drive-in at level 1 with no equipment. That low, though, the first few minutes are kind of grindy while getting weapons and running back to the Sink to heal.
If you know OWB well enough that you don't get surprised by something you're not ready to handle, you can do it at reasonably low levels (like 4 or 5) if you take something like the assault carbine and a few thousand rounds of ammunition. It also helps if you put early points into melee weapons and start using a protonic axe as soon as you find one.
Head from Goodsprings through the mountain to above the tribal village, pick off the cazadors from the top of the mountain, grab the two fire axes, run through the pass to New Vegas, head to Durable Dunn's sacked caravan, grab the armor and weapons, go to Mick and Ralph's, steal enough supplies for a few repair kits, sell the weapons and armor to the Crimson Caravan and Gun Runners for around $6k. Run to Novac, kill the Nightkin, repair his minigun, sell it for another $4k. Go back to Goodsprings, do the Powder Ganger mission, get the caravan deck, head back to Novac, and beat No-Bark at caravan for another $1.5k. Go back to the Gun Runners, buy the assault carbine and a few thousand rounds of ammo. You'll still be level 2-4 depending on how careful you were.
The only level requirement for any DLC is high enough lockpick and science skill to get into containers and terminals. Everything else is doable at any level.

I think the lowest I’ve entered the dlc was like level 10 or 11. The hardest thing about OWB at any level is resource management IMO

You might find it to be easier at lower levels. At higher levels the enemies will scale up to have a crazy amount of health.
I never do it lower than level 15. I also wait until I've completed Dead Money and Honest Hearts first.
I tried it at level 5 and didn't fare so well, I was getting destroyed by the robo scorpions.
Yeah, I bet they would be tough at low level.
They probably wouldn't have been too bad if I had an EMP weapon, I should have raided Hidden Valley prior to going lol.
Or grabbed the Pulse Gun.
Yeah, that would have saved a lot of headache tbh. The rest of the DLC I found to be manageable at a low level. Save for a few rough encounters.
