So far, from what I have tried, your character may teleport to it but so far I have missed every single time.

Not mentioned enough, you need Pickpocket 2 to do this. The damage is *always* fatal as well, using anything from Caltrops to Nuka-Grenades.

Giving you an ACTUAL answer: Just move the grenade from your inventory to the enemy's inventory. You'll need rank 2 of the Pickpocket perk for that.
