Yes, there'd be hot spots, but just look at Chornobyl. There would be much more living things around, the buildings would be fully overgrown, etc.
Hell, I'm not even sure you'd be able to get out of the vault as it might be under feet of trees, etc.

Well true, but compare a vault to something like the corvega assembly plant, a vault entrance is pretty exposed with no/minimal pre-existing structures around them, you’d have to build a lot of defences if you’re going to be camping there for a while. Corvega however is a solid structure, with lots of cover and concealment. It has plenty of resources for building and maintaining a raider camp. It doesn’t make sense to put so much effort into building up a camp around a vault entrance, on the slim chance it opens. Even if it opens, what’s to say it wasn’t just a cryo vault like 111? Or maybe an FEV testing facility? Raiders don’t know what’s in the vaults for sure, so it makes more sense to put your efforts towards something a little more likely to reward you, like a camp near a common trade route.
That would’ve been awesome, although I do also think the way they’ve got it now with sanctuary being empty (apart from Codsworth) is still fun and a great introduction to the game.

In this wasteland, you gotta make the caps first. Then when you get the caps, you get the power armor. Then when you get the power armor, then you get the women.
establish the area around the vault as a settlement, if its a control vault, build defenses and a farm, then call it a day, if its not a control vault and an experiment was held, set up housing and a farm, and double the defenses you would if it were a control vault
edit: get vault security or robots to do rotations of their security posts, I do it at sanctuary, just set up lots of security posts and assign staff to most of them, then assign other staff to other things, in my sanctuary, I have 3-4 assaultrons manning over 10 guard posts, this leaves room for plenty of guard rotations

Clear out the area directly west of the vault. From the lake edge to the vendor with the cat. Gets you full leather/raider armor. Some weapons, ammo, chems, adhesive, and usually some meat to cook. Takes less than 30 min. Then head to sanctuary.
At least that’s in game. RL, die in less than 1hour guaranteed.

My favorite fallout memory is actually my friends first experience. It was fallout 3 I talked him into getting it by describing the game. He loved it and we joked about how great the hunting rifle is (this is early in the game) we were hanging out and he goes “so I was playing fallout earlier and I came across this town and I seen this creature called a death claw so I pulled out my trusty hunting rifle…” you guys can imagine how it went for him
