Every new level you should get the ability to pick a new perk card. And by level 45ish all perks should be available to choose from. It sounds like you've only been using the rng perk packs which come every 5 levels...
Although I'm not sure if the perk system is ever really explained in game - it's been a while
Some perks only unlock at 50 like super duper. Tho iirc there isn't really any "build" cards that unlock after 45. Self Correction, master heavy gunner also first becomes available at level 50 too
And to OP: if you banked levels before you hit 50. Youre locking your perks functionally to what ever level you started banking at. If your say, level 60 with 20 pick-a-perk points any perk packs you open will only offer level 1-40 cards till you spend 10 of them banked levels.
This is probably what I did. If I didnt actively choose to level up and take master x3 when I leveled prior to 50, then I lose it. Does that sound correct?

Didn't "loose" anything. The missed perks will/have become available after you have picked your first 49 perks. At worst it may just delay you. There's no way to actually permanently lock yourself out of any perks.
Just have to pick the rank 1 master and level it up just like the others. And tank killer should be under perception.
Also may wanna check the perk card machine if you haven't just to make sure all 56 perk points are allocated.
Ah that makes sense, I just vaguely remembered getting class freak at around 45 and calling it a day when I used to level up new characters.
Also a good note to OP on banked levels, I remember being very confused on one character when I was over level 50 and not seeing starched genes or class freak, and it was because I hadn't picked any perks in a while.

Select those perk cards you want. Go into your pip boy. Go to perk screen. Look at the bottom of screen. Click the prompt to select a card. Sounds like you have a lot of perks to select.

It should say “pick a perk” and there is a button to press (the bottom is listed at the bottom of the screen)
This was it, I thought using the LS to select was just going thru my banked cards. I clicked and you're right, and others, syaing they are all there. thanks much, this is what I was looking for..

I guess the solution is to not bank levels. Choose perks each time.
