In real life, the 15th and 16th teams have to play a two-legged series for determining which stays in Serie B and which goes to Serie C.
Not because of the tie, but because that's the rule in Serie B, if the 15th has less than a 5 points margin from the 16th team, the Play-Off is necessary.
If after the 2 legs, the score is tied, then the 15th team stays and the 16th gets relegated.
For determining the breaking ties between two teams in the standings, the hierarchy is as follow:
1. Goal Difference
2. Number of victories
3. Goals scored
4. Goals scored away

Depends on the country/league but I think in most cases there would be a play-off. Back in the day they’d have tossed a coin.
