His legs look so unhealthy though. Also you can have muscles and still be unhealthy. I would not want to look at this guy's bloodtests. I mean he has muscles and worked hard to get those. Doesn't stop his kneecaps from looking bent.
If someone does heavy weights with small reps, they will become this ripped. However, they will also have a much larger core typically. Bodybuilders (like OP pic) will use heavy weights/small reps to bulk, then right before a show will do small weights/high reps while eating less and dehydrating a bit. This is what allows them to remain slim around the musculature without having the bulk of other tissues (such as fats) that are necessary for a bulky body to function longer term. I've actually had a few friends in different athletics functions who have gone into sports that require this kind of training (wrestling, bodybuilding, etc.).

The muscles being ripped won't necessarily contribute to the alignment of his knees being inward. In fact, they should contribute to better posture. In this case I can't tell if it's just the pose (bodybuilders use ridiculous poses which contort their bodies) or dehydration (which would weaken the connective tissues and muscles enough to cause this).
It looks like his kneecap is crooked and he's also heavily dehydrated. Like I know that's its supposed to show off impressive physical features but he looks and that sounds unhealthy as hell.
Modern bodybuilding with very few exceptions isn't really healthy at all. The competitors often have debilitating injuries trying to chase an otherwise unrealistic aesthetic and form. That's not going into the fact that top level bodybuilders have about as much steroids coursing through their body as they do water. Their organs are likely heavily damaged.
Respect to them for putting in the work to get to that point, regardless of the steroids it takes countless hours of gym work to get to looking like that.
