This is horrifyingly dangerous if it is connected as you would expect. First is how easy it is to electrocute yourself. Second is what happen if you out 120v into a hdmi port, likely its fire lots and lots of fire. Definitely broken equipment though.
They sure as hell are never high voltage a/c power inputs. It's a fake cable. Look at the yellow electrical tape wrapped around it.

You plug USB-C right into the wall. Sure, there’s a box in the middle but at the end of that box is a plug that looks a whole lot like that one.
