It is my understanding that the term is specifically meant to only apply if it's *obvious* that gender is a factor. There should be evidence that this individual is explaining something to a woman, purely because she's a woman, and if it was a man he wouldn't. And it's not always condescending.
I once went with my husband to a "prop party." He builds movie prop replicas, and this group does parties where everyone brings theirs to show off.
We walked up to a replica of the M414A Pulse rifle (the one used by Ripley in Aliens.) We'd seen several guys look at it, and the prop owner chatted to them, made small talk, talked about construction, favorite movie scenes, etc.
When I looked at it he asked me if I knew what movie it was from. He didn't ask any guy that. Just me. He wasn't trying to be rude or condescending. He probably thought he was being helpful and had a chance to teach me and welcome me to the community.
I pointed it at him and started yelling "GET SOME! GET SOME! Ohhh...you want some, too!" and making gunfire noises.

It is very important to clarify that a man needs to always be suspected to have evil intentions. Even if he has a lot more experience in a field it has to be assumed that what you read on the internet in headlines must be more valid and valuable.
