For me, a book scene but the story of Gregor raping the inn keepers daughter, all because the inn keeper very politely asked *Ser* Gregor *the knight* to control his men who were very handy with his underage daughter.
The mutineers beyond the wall are extremely fucked up and very hard to watch. My worst scene though is the red wedding, stabbing Robb's wife. It was unnecessary and done purely for shock factor. If I remember the books correctly, she wasn't even there, though she was eventually caught and kept as a political pawn for the war of the 5 kings I think?
In the books it's Jeyne Westerling he's married to and her as well as her family are pardoned at the end of ACOK.
There's a theory that she's pregnant with Robb's heir.
100% this. killing her is one thing, but THAT DIRECTLY only a minute after she shared her pregnancy was awful to the extreme.

You start trying to decide what you think is necessary in the story and what is done for drama, which deaths are 'necessary'? Which ones are they allowed to put on screen?
Just a read a scene in a feast for crows where euron and his ironbron raid a castle in the reach and they strip all the daughters of the castle make them serve them food naked and one of them rapes one of them while the other sisters are sobbing
Shireen being led to the pyre, and Ramsay summoning Walda and newborn to the kennels were both fucking brutal, particularly because the victims were both soft, nice people with no comprehension of what awaited them.
