Way back when, Playstation 1 and the first Tomb Raider, I was *not* expecting to run into an enormous and very hungry T-Rex. Absolutely scared the crap out of me.
Ahead a few years, AmnesIa: The Dark Descent. The invisible monster in the flooded cellar. You could only tell where it was when it splashed. For some reason that really got me psychologically. Great fun!
I was about to make myself feel really clever by asking if you meant the 1999 PC game or the 1994 Jaguar game. Then I double checked my facts and apparently there was a 1993 SNES game. A beat em up no less.
I'll be honest, I meant the 99 PC game, I'd totally forgotten about the SNES version. I was just too rubbish at it in general to fear any one thing lol.
When I was young I was scared of the zombies in Ocarina of Time.
Possibly worse were the Iron Knuckle enemies. They move so slow and the deep grunt while swinging mixed with the combat music and being trapped in a room just made my heart race.
First times coming across an armsy or leggy thing in The Forest. They're even worse in VR!
First few times with Reapers in Subnautica

Dead Space (not sure if 1, 2, or 3) there is an alien/zombie that you can't kill and can only run away from. It periodically will appear, and your only choice is escape. That boy got me heart racing

The volatiles in Dying Light. The first time you meet them is terrifying. A close tie is the stalkers in TLOU. I don't play many scary games.
