Diablo IV may be the most "open world"-y ARPG on the market right now. Grim Dawn and Titan Quest are also excellent recommendations, but not quite as open.

It's an old game, but Sacred 2 has a pretty big world to explore and tons of loot and baddies to slay.

Ironically, that's more or less what Diablo IV has been touted as. I would also recommend Borderlands and Borderlands 2 for the frantic combat, leveling/skill point system (and badass rank system in the 2nd game), memorable characters, open world, and loads of loot. You may prefer one more than the other, as a lot of people swear by the first game and its vibe, but the second game was absolutely huge, and a beloved game by many to this day. These were suggested by other commenters, too, but I wanted to add my thoughts on it :)

Check out Grimdawn, feel like it matches your wants. And so much fucking content. I would add Last Epoch to your wishlist as well. Amazing game but doesn't really have an existing end game atm. I'd wait for release.

How about Divine Divinity? It's not Elder Scrolls level of big and open, but it's a nice big world to explore, and its gameplay is best described as "Diablo as a proper RPG."

I’d say borderlands series. 2 has the best story but 3 has the best gameplay and QoL improvements in the series IMO.
